The Facilitator of the Wraparound Program will work on a team providing services to children and their families / caregivers involved in the Los Angeles County foster care system using a Wraparound services delivery model. This model maintains that team members assist high need children, and ensure their success in school, home, and other communities with the ideal goal of maintaining the child’s involvement in community settings. Using information regarding the family’s strengths, needs, history, cultural identity, and past traumatic experiences, the Facilitator empowers the family and child (or youth) by highlighting their strengths, assets, cultural background and discussing the youth’s identification with cultural norms, values, beliefs and practices. In this role, Facilitators are responsible for : 1) convening the team and ensuring the continuity of care and that all identified services in the Plan of Care are provided, 2) ensuring the application of family-centered practice by the entire team, 3) serving as the contact point for children, families, service providers and the community, and 4) ensuring there is continuous input and feedback from the family and service providers.

Essential Responsibilities

Provides mental health assessments, crisis intervention and case management services through a Los Angeles County Department of Mental Health Services contract. Develops CFT meeting agendas, facilitates meetings with the family and child (or youth), and encourages the respective youth and family to lead scheduled meetings. Provides support and coaching as the child and family take lead on facilitating their own meetings. Coordinates and schedules staff and family engagement meetings. Outreaches and engages formal and informal supports. Facilitates debrief meetings with Wraparound team members, including the CSW and / or DPO. Uses the Integrated Core Practice Model when navigating the CFT Meeting. Leads the child and family teams to work directly with the children and family / caregivers per the Plan of Care in their respective environments, including but not limited to in homes, out-of-home placement(s), schools and / or in the community to help achieve permanent placement for the child. Ensures timey completion of required paperwork, record keeping, and performs other, similar paraprofessional job duties as required. Completes other duties as assigned.

Required Qualifications

Bachelor’s degree in Psychology or related field from an accredited college or university is required. Two years of experience working with severely emotionally disturbed youth and DCFS / Probation consumers is preferred.

Working Conditions

Employee regularly works out in the field and in an office environment. Employee may be required to work flexible, evening and / or weekend hours for time-sensitive projects.

Maintain a high level of concentration and attention to detail for extended periods of time. Maintain a high level of ethical and professional standards in accordance with agency policy.

Physical Demands

Ability to talk or hear in order to give and receive information and instructions. Ability to stand, walk, sit, use hands to finger, handle or feel objects, tools or controls Ability to reach with hands and arms. Ability to use computer keyboard up to 50% of the day. Visual acuity, ability to adjust focus and peripheral vision for work on a laptop or other computer equipment. Lift and / or move up to 25 pounds. Able to use telephone, office copier, calculator, fax machine, computer printer and scanner, or other equipment as required.
